US Electrification Products Sales Training Forms

This page provides helpful forms meeting the needs of trainers and customers



Our ABB US Trainers have forms that are needed to be completed in order to provide the learners in their classes with either CEU credit, PDH certificates or Certificates of Completion. Below are links to the forms to be completed to request those certificates.

The ABB US Training Team also assists the sales organization through hosting of webinars. A request form is located below and is needed to be completed if assistance is requested from the Training Team.

Forms required to be completed by CEU trainers when planning and delivering CEU courses

Course planning form

This form needs to be submitted 30 days before a scheduled presentation at customer location.
Class checklist form

This form should be reviewed by you before you go to customer location for presentation but can submitted after presentation completed.
CEU credit request form

This form is to be completed to request certificates for learners that want CEU credit.
CEU Trainers - Required BIO Form

IACET Requires Biographical Information for all the authorized trainers who will be delivering CEU credits. Please click here and complete the online form.

Certificate requests

OCAL training certificate

Complete this form after conducting OCAL Installation Training for certificates and ID cards to be sent to the group trained.
Certificate of participation

Complete this form for any Lunch & Learn presentation where the customer would like to receive certificates.
PDH certificate request

Complete this form if you have delivered a presentation to a group that is looking to receive PDH credit.

Complete this form if you would like ABB US Training Group to assist you in scheduling a webinar.
On-Demand Distributor field training

This form is to be completed to request training to be done at a distributor location during one of the available weeks shown on the list.
